Use a fictional contact name for this demonstration.

Use a fictional email such as alex@example.com. No email will be sent.

Describe the technology, its purpose, and why an exception is needed.

Identify the affected task and applicable WCAG criteria, if known.

Describe who cannot complete the task and how the barrier affects them. Do not include personal health information.

Describe the number of users, frequency of use, and whether the service is public facing or internal.

Describe the alternative way to complete the task, who provides assistance, and expected response time.

Enter the planned start date. Use YYYY-MM-DD, for example 2026-10-15.
